The annual Universal Health Record form, along with a current shot record, must be signed and stamped by a Doctor and handed in September at the annual Meet and Greet. When your child has their annual checkup, please submit a new universal heath form to update our records during the school year. ALSO, please download and sign the parent packet and return it with your medicals the day of the Meet and Greet on September 9th. These are required for your child to attend the first full day of school. Please do not wait for your child's well-check up. Shots can be updated throughout the school year. All forms are provided on this website. A Mandatory Flu shot is due by December 1st.
September Tuition may be turn in at this time as well.
MEDICATIONS: All children that require an Epi Pen and Medications for severe allergies must submit an Allery Action Plan filled out by Doctor. All medications must be in the sealed original box in full -- no individual tablets. Check the expiration dates please. EPI PENS must be in the original prescription box with the prescription label. If the prescription requires two epi pens then you must turn in two epi pens to the office.
